Choose your preferred appointment date and time slot.

Patient Information & Medical Intake

Complete the required patient details and medical intake form. Optionally upload reports, scans, or prescriptions.

Confirm your appointment details, patient information, and any additional notes.

Submit Booking

Submit your appointment request for processing.

Make Payment via Cash App

Send payment to Cashtag $TechV and upload a screenshot as proof.

Payment Policy

Payment is required before appointment confirmation.

Cash App ($TechV) is currently the only accepted payment method.

Payment must be received within 48 hours of booking.

Vincent Ervin II will verify your payment within 24 hours.

Refunds are available up to 24 hours before the scheduled appointment.

No-shows may forfeit their payment and require rebooking.

Emergency appointments may have different payment terms.

Patient Conditions

Valid government-issued ID is required at check-in.

Arrive 15 minutes before your scheduled appointment.

Bring any relevant medical records or test results.

Inform the coordinator of any allergies or medications.

Cancellations must be made at least 24 hours in advance.

Telehealth patients must have stable internet and a working camera.
